Annual Hormone & Longevity Membership

The Lilivy Collective is Dr. Stephanie Sublett’s annual membership for women who want more than a rushed appointment and a list of generic recommendations. It is a place for thoughtful conversations, advanced evaluation, and physician-led care that considers the full picture: hormone health, perimenopause and menopause, metabolism, sexual wellness, and the long-term health decisions that shape how you feel every day.
At Lilivy, care is personal by design. Dr. Sublett brings the clinical depth of a board-certified OB/GYN and Menopause Society Certified Practitioner to a setting that feels calm, private, and highly considered. Your plan is built around your history, symptoms, priorities, and the life you want to feel present for.

A 10-minute appointment rarely leaves room for the questions that matter most. Many women spend years moving from one concern to the next, trying to connect symptoms that may involve hormones, sleep, stress, metabolism, sexual health, aging, or changes in body composition. The Lilivy Collective was built to make space for the whole conversation.
Dr. Sublett created this membership because women deserve care that is both medically rigorous and deeply personal. She believes in clear answers, meaningful follow-up, and decisions made with the benefit of your full health history. The result is a more intentional model of care, designed for patients who value expertise, access, and continuity.
The Collective, Defined.
The Lilivy Collective is an annual concierge membership created for women who want a more complete relationship with their physician. It offers the time, continuity, and clinical attention needed to address concerns that often overlap: changing hormones, sleep, energy, mood, metabolism, sexual wellness, body composition, and the health questions that become more important in midlife.
This is Dr. Sublett’s signature care model. Rather than treating one concern in isolation, she works with you to understand the patterns beneath it, identify the factors that matter, and create a plan that can evolve as your body and priorities do.
For some members, that plan centers on perimenopause or menopause. For others, it may focus on hormone-related symptoms, weight-management support, sexual wellness, metabolic health, or proactive longevity planning. Every plan begins with the same foundation: a physician who listens closely, looks carefully, and stays involved.

The Collective provides a structured, year-long framework for your care. It is designed to give you a clearer starting point, more consistent physician involvement, and a plan that is refined as your needs change.

Your membership is led by Dr. Sublett, with care designed around your health history, symptoms, priorities, and clinically appropriate next steps.

Your care begins with a detailed Wellness Strategy Consultation, creating time to understand the concerns that brought you in and the goals that matter most to you.

When clinically appropriate, Dr. Sublett may recommend lab work, body composition analysis, PNOĒ metabolic testing, or other evaluation tools to inform your plan.

Your membership includes ongoing care through the visits, follow-up, and approved communication access outlined in your final annual agreement.

Your plan may include support for hormone health, menopause symptoms, metabolic and weight-management goals, sexual wellness, nutrition and movement strategies, and long-term health planning.

For Women Who Want to Feel More Informed, More Supported, and More In Control
The Lilivy Collective is for women who are ready to take their health seriously and want a physician who will do the same. You may be navigating the early signs of perimenopause, adjusting to menopause, questioning whether your symptoms are hormone-related, or simply looking for a more proactive approach to the years ahead.
You may recognize yourself in the following concerns:
•Your sleep, energy, mood, concentration, or cycle has changed, and you want a more complete evaluation.
•You are experiencing hot flashes, night sweats, vaginal dryness, changes in libido, or other menopause-related symptoms.
•Weight changes feel more complex than diet and exercise alone, and you want thoughtful metabolic guidance.
•You want to discuss hormone therapy, sexual wellness, body composition, or longevity planning with a physician who understands women’s health.
•You value privacy, access, and a long-term care relationship over a one-time visit.
The Collective is not an aesthetic membership or a collection of services. It is a physician-led commitment to your health over the course of a year.
The First Step
Every Collective membership begins with a private, 90-minute Wellness Strategy Consultation with Dr. Stephanie Sublett. This is a dedicated appointment for reviewing your health history, current concerns, goals, and the parts of your life that may be affecting how you feel.
You will have time to discuss what has changed, what you have already tried, and what you would like to understand more clearly. Dr. Sublett will talk through appropriate next steps, including whether The Lilivy Collective is the right framework for your care.
Testing, treatment recommendations, and any prescriptions begin only after membership enrollment, if The Collective is determined to be appropriate for you.
